Bowdon Baptist Church cares for each child participating in our ministry and their families. The questions in this document are asked for the sole intent of benefiting your child. The questions will help us provide the best experience and safest environment for everyone involved. Our church leaders and ministry volunteers respect your family’s right to privacy. Any information shared from this form is communicated directly with those caring for your child and only on a need to know basis. The information is kept in a secure location, but made available to all who may work with your child. You have the right to access the information at any time by contacting our Minister of Students and Families.

My child has my permission to participate in all activities of the Bowdon Baptist Church, Inc. and to be transported by church van/bus or private vehicle when necessary. I understand all events will have adult supervision. In consideration of the benefits to be derived from these activities, I hereby voluntarily waive any claim against the furnishing transportation to and during any event. I further agree to direct my son/daughter to conform to the fullest with the directions and instructions of the sponsors in charge. My child agrees to obey all bus safety rules which includes, but is not limited to: wearing seatbelt, sitting at all times the bus is in motion, and using appropriate voice. This consent and release is in effect until I give the Bowdon Baptist Church, Inc. written notice to the contrary.
